Students completing Placements in schools or other agencies
To ensure that you complete the correct forms, please read ALL of the information on this page before completing the appropriate Forms. OESC is not responsible if incorrect forms are completed and delays occur as a result.
This section is for college and university students who require a Police Record Check for their placements. The forms below may be downloaded and submitted to OESC.
School Boards and other agencies require students placed in Ontario schools and agencies to complete a satisfactory Police Record Check screening prior to having direct contact with students and other vulnerable populations. Without a satisfactory Police Record Check students will not be allowed to participate in their placements.
You will be issued a wallet-sized Identification Card authorizing you to have contact with students in Ontario schools and with other vulnerable persons IF you have no police record OR if you have a record and it is determined by the OESC adjudicators that you are not a risk to these individuals. This card must be signed by you and made available to school and agency officials when requested, together with a valid government issued photo identification document (Driver's Licence, Passport).
Applicants who are refused authorization will have an opportunity to provide an explanation, and will be advised of the reasons for the decision of the OESC adjudicators. Details of a police record will be shared only with the applicant and OESC administrative staff and adjudicators. Your College or University will be advised only of your status.
Processing takes approximately 4 weeks (20 business days) AFTER receipt of an accurately completed consent form. Once your Police Record Check has been satisfactorily completed, you will receive, by mail, a wallet-sized Identification Card authorizing you to have contact with students in Ontario schools or with other groups, where applicable. This card must be signed by you and made available to officials when requested, together with a valid government issued photo identification document (Driver’s Licence, Passport). Some community college programs also request that students obtain a written report at this time. This can be obtained by checking the appropriate box on the payment form. If you are unsure of the requirements of your program, please check with your program coordinator. OESC is not responsible if the incorrect documents are requested.
The OESC ID card is valid for one school year. If you require a Police Record Check in subsequent years of your college/university program you will be required to undergo a new check each year.

Please complete the Consent Form in full, sign it and send the original with a completed Payment Form and payment (Money Order, VISA or MASTERCARD) directly by mail or courier to the OESC at the address below:

If you are sending the forms by regular postal mail, please factor in the time it will take for the forms to be delivered to our P.O. Box and then forwarded to OESC for processing (3 business days).
If you are sending the forms by courier you MUST use Priority Post or Express Post only. Other couriers (e.g. UPS, Purolator, FedEx) do not deliver to Canada Post P.O. Boxes. We recommend that you keep a copy of your documents for your reference.
You MUST send the original forms to OESC. Your Consent Form is a legal document. Faxes, photocopies and electronic versions are not acceptable. Do not send CASH or personal cheques or your forms will be returned to you, delaying processing.
(If you complete these forms you will receive an OESC ID card which will be valid for the 2009-2010 school year (i.e. September 1, 2009 August 31, 2010)
